Welcome to CMMC! To ensure your admission and registration process goes in a timely manner, we encourage you to read the following and gather necessary information before your visit. CMMC has two convenient areas that you are able to register depending on your needed service/procedure:

Located inside of the Emergency Room, a registrar is conveniently located to ensure all necessary information is processed quickly.

Consider and bring the following:
Bring important information – driver’s license, social security card, insurance cards, other personal identification and necessary insurance forms.
Also be sure to bring any information your physician provided to you.
Be sure to arrive 10-15 minutes before your scheduled appointment.
